NDIS Support Coordinator / Psychosocial Recovery Coach

Position Purpose:

Support Coordination and Psychosocial Recovery Coaching focus on supporting participants to direct their lives, not just their services. By providing flexible and adaptable services, we aim to assist participants in building and maintaining a resilient network of formal (such as mainstream health services) and informal support (such as families and caregivers). This position is also designed to build capacity and resilience through solid and respectful relationships to support people with psychosocial disabilities to live whole and contributing lives.

About the role:

In line with the NDIS guidelines, you are required to undertake the following:

  • Welcome and engage the NDIS participant and any support to ensure they are fully aware of the role of Support Coordination / Recovery Coaching.

  • Support the NDIS participant to help them understand and implement their NDIS plan.

  • Ensure the NDIS participant has a service agreement and understands its contents, terms, and conditions as part of the engagement process with all service providers.

  • Act as a central reference point to support and build the NDIS participant's capacity in coordinating their NDIS-funded, mainstream, and informal support.

  • Promote and support NDIS participants in exercising choice and control when signing service agreements with service providers and utilising their support services.

  • Working alongside the NDIS participant and supporting them in connecting with the most suitable support services enables them to achieve their key goals in the NDIS plan.

  • Coordinate a broad range of service providers and ensure a team approach to addressing the support needs of the NDIS participants / external referrals.

  • Ensure all services engaged in support of the NDIS participant have a shared understanding of the elements of the NDIS Plan and that all service providers work in a collaborative and integrated way.

  • Conduct regular reviews of the support's effectiveness and provide detailed progress reports to the NDIA as required within an expected timeframe.

  • Seek feedback from all relevant stakeholders (including, but not limited to, the participant, their family and caregivers, supervisor, and colleagues) and always strive to refine practice frameworks.

  • Meet all data entry, administrative and reporting requirements for NDIS regarding the provision of Support Coordination and Psychosocial Recovery Coaching.

  • Ensure Policy and Procedures are adhered to and always align with Stepping Stone's values.

  • Maintain up-to-date knowledge of NDIS systems, pricing arrangements, limits, practices, and legislation, knowledge of the Mental Health systems, NDIS-funded and non-funded mainstream services, and formal and informal support.

Selection Criteria:

Must-Have:

  • NDIS Worker Screening Clearance or can acquire one.

  • A tertiary qualification in peer work or mental health (minimum of Certificate IV in Mental Health Peer Work or Certificate IV in Mental Health) or equivalent training.

  • Knowledge, understanding, and preferably lived experience of the impact of severe mental ill-health, recovery, well-being, and capacity to work with people with multiple and complex support needs.

  • Knowledge of the NDIS and navigating the support system, including accessing relevant community resources.

  • At least two years of experience in the SC/ PRC role or a similar position in mental health-related work.

  • Highly developed communication, interpersonal, and organisational skills.

  • Skills and experience in coordination, collaboration, facilitation, critical thinking, evaluation, and problem-solving.

  • A current Open Driver's Licence (QLD).

  • Excellent computer literacy and IT competencies with Microsoft programs and Client Management Systems.

  • Excellent time management skills

Southside Clubhouse Mental Health Support Worker

Position Purpose:

The Mental Health Support Worker position will work to deliver Clubhouse services and supports in a manner designed to empower members and to assist them in achieving their individual goals.

This support will be delivered according to the guidelines and principles of various frameworks (including the Clubhouse model and NDIS), and as such the role of the Mental Health Support Worker position will need to be flexible and adaptable in their approach to each individual members’ needs.

About the role:

In line with Clubhouse International’s Model of recovery oriented practice:

  • Work in a generalist role to assist members to identify their goals, and provide individual support such as

    employment, education, housing, advocacy, and assistance to access external services.

  • Facilitate personal recovery and wellbeing through the provision of supports to members consistent with

    their defined goals and aspirations.

  • Facilitate the work of the unit, engaging members in all aspects of unit work with the aim of increasing members’ self-esteem, concentration, stamina and social skills.

  • Developing and maintain meaningful work opportunities in the unit for members.

  • Assisting in the outreach program providing phone, email, mail, home and hospital visits.

  • Presenting to community groups regarding mental illness and the Clubhouse model.

  • Ensure that all member information is entered into the Member Database in a timely manner

  • Meet all data entry, administrative and reporting requirements for NDIS and other funding bodies.

  • Undertake relevant training and professional development, including regular supervision, appropriate to the

    primary work of the service.

  • Provide support, training and absence coverage for members as part of the Transitional Employment

    Program.

  • Participate in the Social Recreational Program both within the Clubhouse and in the community.

Selection Criteria:

Essential Qualities:

  1. Knowledge, understanding and/or lived experience of the impact of serious mental ill-health, recovery, well- being and capacity to work with people with multiple and/or complex support needs.

  2. Knowledge of the NDIS, and how to navigate the support system including the ability to access a range of relevant community resources.

  3. Demonstrated experience and skill in programs for people with a mental illness and ability to manage these programs in a way that ensures maximum consumer participation.

  4. Highly developed communication, interpersonal and organisational skills.

  5. Skills and experience in advocacy, collaboration, facilitation, evaluation and problem solving.

  6. Possess a current open C-Class Drivers Licence valid in Queensland.

  7. Sound IT competencies with Microsoft programs and Client Management Systems.

  8. Hold or be eligible NDIS Workers Screening.

Desirable:

  1. An appropriate University degree

  2. Interest in/knowledge of the Clubhouse model
